1 Short Form = This form is for Out of State Merchants who already have a business license in their own Home State, but will not need an AZ Privilege Tax ID since this is the only show they will do in Arizona this year. Please be sure to include a copy of you business license and a short explanation stating that this show is a once a year show (name of show, date and location) That way there is no excuse for mix up for AZ State Employees.

2 AZ Joint Tax App = This form is for new Arizona Resident Merchants who will need to apply for a business License + Tax ID for this show.

NOTE: If you already have an AZ Business Lic + Tax ID, you should only need to notify the Dept. of Revenue that you will want to add the City of Goodyear to your Tax ID.
